HCC Manager - Risk Adjustment (Remote, MUST live in South Carolina)

Job Summary

The HCC Manager is responsible for the management of the Hierarchical Condition Category and Risk Adjustment Coding Program providing oversight of concurrent, prospective, and retrospective review of ambulatory medical record clinical documentation to ensure accurate and complete capture of the clinical picture, severity of illness, and complexity of the patient.  The manager utilizes knowledge of official coding guidelines (ICD-10, CPT, HCPCs), Hierarchical Condition Categories (HCC), M.E.A.T (Monitored, Evaluated/Assessed/Addressed, Treated) standards, Risk Adjustment Factor (RAF) scoring, and AHIMA/ACDIS physician query brief.  Includes but is not limited to managing the risk adjustment provider Clinical Documentation Initiative process, educating providers on the importance of diagnosis specificity; developing, presenting, and disseminating provider communication and other activities related to clinical documentation. 



Requirements

Minimum Qualifications

Minimum Education: Bachelor's Degree in Business Administration, Health Information Management, or related discipline. 
Minimum Years of Experience: 5 Years of related coding experience.
Substitutable Education & Experience: A Bachelor's Degree can be substituted for the one of the following combinations: Associate degree with 2 years of additional directly related work experience; 
High School Diploma or Equivalent with 3 years of additional coding management/supervisory or educator experience.
Required Certifications/Licensure: Candidates must have one of the following certifications: Certified Risk Adjustment Coder (CRC), Certified Professional Coder (CPC) or Certified Coding Specialist Certification (CSS).
Those without a CRC Must Obtain Within the First Year of Entry into Role.
Required Training: None.
 

Essential Functions

  • Concurrently, prospectively, and retrospectively reviews medical records to identify unclear, ambiguous or inconsistent documentation.
  • Facilitates workflows that support the clinical picture/severity of illness/complexity of the care rendered to the patients.
  • Utilizes approved resources to determine the appropriate ICD-10-CM, CPT, and/or HCPCs and ensures documentation in the medical record follows official coding guidelines, internal guidelines, and AHIMA/ACDIS physician query brief.
  • Provides ongoing feedback to physicians and other providers regarding coding guidelines and requirements, including education and support for improvement in HCC coding and RAF scoring. Assists with educational in-services for physicians, other providers, and clinic staff relating to clinical documentation compliance as well as policies and procedures related to billing.

  • Recruits, selects, and develops personnel to support the areas of responsibility.
  • Maintains complete confidentiality of patient information
  • Maintains competency in evolving areas of coding.
  • Collaborates with other LMC departments as needed.
  • Performs all other duties as assigned.
